Band 5 Family Wellbeing Nurse
Our client is one of UK's leading healthcare providers and they are currently seeking for a dedicated Band 5 Family Wellbeing Nurse. You will be working in an exceptional surgery within the Braintree, Essex area
This special surgery is committed to providing patients with a full general practice service and access to a range of health professionals. They offer a range of health services and information as well as advice and treatment for a range of illnesses and minor injuries
**You will be a Registered Nurse (RGN, RMN or RNLD) with a minimum of 6 months post-graduate experience**
As a Nurse your key duties include:
· Working alongside Health Visitors and the wider skill mix team you will have a real opportunity to improve the health and wellbeing outcomes for children and families in the community, for the community and with the community
· To work with the Health Visitors to offer every family a universal health visiting service to support all aspects of a child’s development and family’s health from ante natal until the child is 5 years of age
· You will be responsible for delivering the Healthy Child programme, working with multi-professional teams to facilitate multi-agency and interdisciplinary working
· You will promote and maintain an effective service to clients, using the assessment framework, communication, negotiation and collaboration
· You will work within the Children Act 2004 and act as an advocate for the voice of the child. You will assess, plan, implement and evaluate packages of care
· You will identify concerns and report and refer those directly to social care or with Clinical Leads, Named Nurse and Safeguarding Supervisors
The following skills and experience would be preferred and beneficial for the role:
· Strong communication and organisation skills
· A flexible approach will ensure you deliver positive experiences to service users at every stage of their healthcare journey
· You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, and Excel
· Have a caring and approachable demeanour in order to gain the trust and respect of local families and children
