An amazing new job opportunity has arisen for a committed Intermediate Care Nursing Sister to work in an exceptional health centre based in the Camberley, Surrey. You will be working for one of UK’s leading health care providers

You will be assessing patients in Accident and Emergency for swift discharge and on the wards assessing patients before they go home with support from Intermediate Care to ensure smooth transfer of care from the acute setting to the community setting and their own homes

**To be considered for this position you must be qualified as a Registered Nurse with a current active NMC Pin**

As the Nursing Sister your key duties include:

·         You will work alongside the therapist's and other members of the Community MDT to support patients to live safely and independent in their own homes

·         You will assess patients in their own home for Admission Avoidance, as well as supporting and assessing them following a fall at home

·         you will work and liaise with the therapy teams to support and assess patients who are being discharged home, either on the wards or in ED

The following skills and experience would be preferred and beneficial for the role:

·         Community experience and have the Health Assessment

·         Excellent communication skills

·         Be able to work in a busy environment, which requires flexibility at times

·         EMIS experience desirable but not essential

·         You will also need to be confident using IT systems
